Osteoarthritis of knee joint is very common age related condition that is caused by increased wear and tear of articular cartilage and menisci. Knee joint is formed by articulation of femur, tibia and patella. There are articular cartilage and menisci between articular surfaces of bones. Function of these are as shock absorber and prevent friction of bones. Due to aging process, inflammatory and traumatic conditions there was increased wear and tear of these protective structures. Osteoarthritis is caused by wear and tear of these structures. Stages of osteoarthritis of knee joints : 1) Stage 0 - Normal healthy knee joint. 2) Stage 1 - Minimal disruption of articular cartilage and less then 10% cartilage loss. 3) Stage 2 - Joint space narrowing, cartilage breakdown and occurrence of osteophytes 4) Stage 3 - Moderate reduction of joint space and cartilage breakdown reaches upto bone. 5) Stage 4 - Joint space greatly reduced and large osteophytes formation. There are two types of flat foot : 1) Flexible flat foot - In flexible flat foot variety foot looks normally curved in dorsal aspect in non weight bearing position, but in weight bearing position foot becomes flat. 2) Rigid flat foot - In rigid flat foot variety foot remain flat in both weight bearing and non weight bearing position. Causes of Flat foot : Most common cause of flat foot is hereditary. But in following conditions 1) Tendoachilles injury 2) Bony fractures 3) Diabetes 4) Obesity 5) Hypertension 6) Rheumatoid arthritis and other arthritic conditions chances of flat foot is high. Symptoms : Normally function of arches is to protect tendons, nerves, blood vessels and other soft tissues of dorsal aspect of foot by properly distributing weight in pressure points of foot while standing, walking, running and jumping. Due to compromise of these functions patient develops pain and swelling in feet, leg cramps and ankle and calf region pain. These symptoms are more in night time. Diagnosis : Flat foot is diagnosed clinically. In rare cases of rigid flat foot and those flat foot involving bony components requires X ray. Treatment : Doctor treat this condition symptomatically by giving NSAIDS medications, vitamin B12 and vitamin D3 supplements and nerve medicine like pregabalin. Sciatica is common condition that is related to compressive pathology of sciatic nerve. It is caused by 1) Herniated disc 2) Disc degenerative disease 3) Lumbar canal stenosis 4) Spondylolisthesis 5) Tumour, cyst and other soft tissue swelling compressing nerve. In obesity, smoking, prolonged office working hours and physical inactivity increases chances of sciatica. Symptoms of sciatica are low back pain, pain radiating to leg, numbness and tingling sensation in legs. In severe condition patient develops weakness in both lower limbs and loss of bowel and bladder control. It is diagnosed by patient's history and clinical examination. Doctor performs nerve stretching tests like straight leg raising test to diagnose sciatica. In some patients doctor prefer to do x ray and MRI.
